IT Financial Risk Assessment (ITFRA) Form

In accordance with professional standards*, Withum is required to obtain an understanding of its clients information technology environment as part of its audit procedures. This includes gaining an understanding of their information systems relevant to financial reporting, including general IT controls, application controls, and aspects of such systems relating to information disclosed in the financial statements. As a result, Withum has established questions regarding the organization’s IT infrastructure, critical financial applications, and controls. The information obtained through this form will not be shared with any third parties. Your responses will aid Withum to better understand your organization and assess the risks, as required. The individual that this form was initially directed to has the ability to add an additional individual(s) to access and complete the form. Access to this form will be automatically removed after 60 days for security purposes.

Information obtained within this form is deemed and will be treated as confidential.
